Breast Cancer Prevention Services for Low-income Women in Dane County
The goal of the Breast Cancer Prevention Services for Low-income Women in Dane County project is to help Access increase the number of low-income women who are receiving recommended mammogram screening through the use of a quality improvement processes, increased case management and outreach services, and patient incentives.

African American Council of Churches, Inc: The Witness Project of Madison

African American Women’s Participation in Breast Cancer Outreach and Navigation
The Witness project will increase educational outreach to at least 300 African American and medically underserved women in Dane County and provide educational support to faciliatate access to clinical and treatment services. We will strengthen accessibility of psychosocial support to African American and medically underserved women. Lay health advisors will teach basic information about breast cancer screening and prevention. We will collaborate and conduct two major events, Hats ON for Breast Health and Pink Ribbon Sundays.

Allied Wellness Center

WE CAN! And SASSY!
The Allied Wellness Center will partner with the Wisconsin Well Women Program to create a shared outreach specialist position who will work in Allied 7 hours per week and at WWWP 13 hours per week. She will promote a salon partnership program called SASSY (Save a Sister, Save Yourself) which will offer salon gift certificates to women who receive their first or overdue mammogram. In addition, she will follow up with women who were involved in the 2009 Komen funded We Can (Wellness Education and Cancer Awareness Network) events to encourage these women to follow all screening guidelines and lifestyle recommendations, and to help them overcome perceived barriers to screening. She will continue our work with the Welcomers to reach African-American and other minority and low-income women in neighborhood settings.

Breast Cancer Recovery

Sharing the Knowledge Conference
Breast Cancer Recovery will host the eleventh annual Sharing the Knowledge: Resources for Recovery conference, November 6, 2010 in Madison, Wisconsin. The day-long conference features experts sharing information on recent breast cancer research, new breast cancer management techniques and survivorship resources. It is designed for breast cancer survivors, caregivers, and health care professionals. Registration fee is $50 and scholarships are available to anyone unable to afford the fee.

Center for Patient Partnerships

Dane County Advocacy Safety Net Project
During 2010-11, the CPP will collaborate with the WWWP to host a series of “Collaborative Conversations,” co-facilitate two subsequent “Community Conversations” with African American women, train WWWP’s new African American Outreach Worker in advocacy skills, and pilot test targeted advocacy services for medically underserved women. We will also provide advocacy training to the WWWP African American Outreach worker and pilot test targeted advocacy services. The WWWP will refer 10-15 women to the CPP for services.

Online Komen Self-Advocacy and Survivorship
This progam will provide on-line training and materials to support breast cancer survivors as they advocate for themselves and prepare holistically for survivorship. Specifically, the programs provide Dane County breast cancer survivors the skills and impetus to advocate for themselves at critical junctures that impact health care outcomes, to take full advantage of available information about diagnosis, treatment, support and aftercare, as they prepare for their “new normal” as survivors. At a minimum, 100 Dane County breast cancer survivors will take advantage of the online curriculum.

Gilda’s Club Madison

Gilda’s Club Madison Program
Gilda's Club Madison has created a free community of support for anyone touched by cancer. We find that over 30% of our members are women and men directly affected by breast cancer. We propose to provide additional networking and support groups to all who are touched by cancer. We seek to advance our Diversity Initiative so that underserved populations will learn about our fee program, and begin in increasing numbers to take advantage of it.

Monona Library Komen Collection
The purpose of the program is to provide easy access on the East Side of Madison and the Monona area to breast health-related materials including books, DVDs and audio books in English and Spanish, plus vetted non-commercial online resources of current information. Key activities include developing the Komen Collection of circulating materials; vetting online sources of breast health information; developing informational packets, as well as online and local resources.

Conference: Do You Hear What I Hear?
The University of Wisconsin Carbone Cancer annually sponsors a fall cancer conference designed for individuals who are involved in treating cancer and educating patients. Now in its ninth year, the 2010 fall conference will focus on cancer communications. A national breast cancer speaker will be engaged to address one of the communication topics.

UWHC Breast Cancer Treatment Services
The intent of the UWHC project is to provide low income women or women who lack the means to pay privately, additional breast cancer screening, treatment and support services. The program will include genetic counseling/testing, rehabilitation service integration into treatment plans, complementary and alternative medicine services, CareWear products, and translation of materials into Spanish.

Wisconsin Comprehensive Cancer Control Program

Breast Cancer Task Force
The Breast Cancer Task Force (BCTF) will be developed to address a decline in breast cancer screening rates. The BCTF will review and analyze relevant screening data, identify priorities within the Wisconsin Comprehensive Cancer Control Plan, and implement a pilot intervention in health communications. It will provide a forum for collaborative priority setting, networking, and implementation of pilot projects.

Wisconsin Well Woman Program

Increasing Breast Cancer Screening for African American Women
The WI Well Woman Program, through the SASSY (Save a Sister – Save Yourself) initiative will increase breast health education and breast cancer screening rates for uninsured African American women with limited incomes who live in Dane County. An Outreach Specialist will be hired to work specifically within African American communities across the County. The initiative includes offering incentives to eligible African American women who receive their mammograms, sharing Outreach Specialist hours with the “Welcomer’s Program at the Allied Wellness Center and collaboration with the Center for Patient Partnerships in facilitating two “Community Conversations.”

Treatment Access Fund
The Treatment Access Fund provides financial assistance to currently lower income uninsured and underinsured individuals in Dane County who are in need of breast related services or have incurred bills related to breast cancer screening, diagnosis and treatment. Expenses for eligible individuals are paid directly to the healthcare provider. Funds may also be used to provide temporary rent and utility assistance for individuals with breast cancer.

Wisconsin Women’s Health Foundation

B-SMART and STAY-SMART: Reducing Stress and Facilitating Coping While Living with Breast Cancer [ Enroll Today ]
The WWHF will offer a 10-week intervention called Breast Cancer Stress Management and Relaxation Training (B-SMART) to women with any stage of breast cancer. It will offer seven 10-week courses throughout the year and partner with the Allied Drive Community Center to provide services at their location. They will consult with specifically trained professionals regarding development of culturally appropriate workbooks and train an Allied community member as co-facilitator. They also seek funding to offer a “Stay SMART” two-day intensive retreat.
